2006 Road Hockey Players Registration

Each team will have a Team Captain who will assume responsibility on behalf of their entire team They will ensure all information is correct as well make payment in full to be submitted to the address provided. This year entry fee is $250 team

You will receive an email notifying you that your ONLINE registration has been received in our office within 24 hours. It will give you specific instructions to be followed.
Please print a copy of this ONLINE registration and submit it with your complete payment to address listed below.
All 7 Seven Players MUST submit a Signed Waiver and Release Form, without it you will not participate
ALL teams will submit 1 ONE cheque for the entire amount owing.
Once Payment is confirmed you will receive another email to this account indicating if your team has been accepted and other IMPORTANT final instructions.

YOUR TEAM Payment Envelope must contain the following and Sealed with Your
Team Name and Division ON Front.
A Copy of your teams ONLINE registration form
Seven 7 Individual Player Waiver & Release Forms.

It must be Hand Delivered within the next 48 hours and Placed directly into the Mailbox:
